Usually it means total from what I've seen, but I'm not exactly sure what the standard is, if there is one. Where I play, the dealers wait and see what the player does after he says that. Some people mean 50k total and some mean 50k on top. Usually if your action immediately follows your words, it won't be a problem.
To avoid the ambiguity, I usually say, "raise to 50k" or I'll just say "raise", get my chips, and say a number (that is the total) as I'm putting them down.
