It would be good to see your positional stats. Also, is this 6-max or full ring?
It looks like you need to play less hands in early position, play more hands in late position, and stop open limping. You might also want to look into c-betting the flop more often.

So yeah, you need to stop open-limping preflop (that means calling the big blind before anyone else has put money in the pot), and start open-raising more in late position. So for example, if you're dealt 88 UTG, you should raise instead of calling. For another example, if you're dealt 98s on the CO and it folds to you, you should raise instead of calling or folding.

I'll try to keep this in mind and play as you recommend. I seem to be doing alright at 2NL, but I just suck when I try to go to 5NL. I can't get positive cash flow to save my life. It's frustrating as hell, cause I'm trying to play the same way.
I know I've got to get better at putting people on a hand, I just can't seem to grasp it. I guess at 2NL it must not be that important, thus I do fairly well, but at a little higher up it must matter more and I'm just getting crushed.

It sort of depends. There are two opposite ends of spectrum between raising being really good and calling being really good. Here I'll briefly describe each extreme, and between there are lots of shades of gray:
1 - Five limpers, most of which don't limp/fold often, all of which are 100bb deep with you, and someone in the button/blinds likes to 3-bet a lot. Here you should almost always call.
2 - One limper who limp/folds a lot, or limp/calls a lot and folds to a lot of c-bets when no one left to act behind you likes to 3-bet a lot. Here you should almost always raise.

As spoon said, you gotta stop limping. Looks like you're playing a lot of marginal hands in early positions and are often giving up the initiative preflop. In general you only really want your VPIP/PFR to differ by like 2-4% or so. I don't play FR but I believe it's pretty common to be running around like 15/13 over all.
