You really need to do this regularly. I refused to do it in any depth for a long time as I didn't really think it would help that much. I spent an entire Sunday doing this (about 6 hours) as Sunday is usually my poker break day.
Sort HEM or PT for an entire month with hands where you won or lost 7bb or greater. I went back 2 months so I wouldn't remember the hand as well. Also use hands that went to showdown to begin with so you can see how well you are doing with ranges.
Open Pokerstove to use for every hand and assign a range for villian for every street of every hand. At first you are really going to suck at this but you'll be surprised at how fast you get better at it.
Doing this was the turning point in moving up for me. Doing it regurlarly makes alot of difference. I've kinda slacked off recently due to time constraints but for a long time I did it before every session on hands from 2 days ago (so I might not remember the hand as well) and I included hands that didn't go to showdown. You will only have 15-25 hands to review before every session so it won't take to long.
I think Spenda made a video on how to do this but I can't remember if it's a FTR vid or a Grinderschool video.

On a side note: It looks like I may have some time to see if I could be a "professional grinder" as the government looks like it's going to shut down next week (I work for the gubmint). If this happens I may start an OP.
