I'm not prepared to say it just isn't meh writing. Just like how the show set it up going "wwooooooooo lookit these burnt boys they could be bran n rickon omgomg" in a tv-like cheese way, cat and the sword could just be the same thing
Besides, if Cat did maim him, it goes against the character that the writers established for her, and they are bad and should feel bad. She is level-headed and does what's best for her children, full stop.
Seriously, her even asking for the sword is bad writing. When she learned the Theon took over Winterfell and her two kids and kill Roderick, she still wanted to talk to him. Her asking for the sword was just TV being TV, looking for a climax wherever possible.
