My understanding is that it means the most that can be bet by a player in any hand. So even if you have $200 at the table, if the cap is $20 and you went AI, you would only bet $20.
Cap and spread limit are two iterations of poker that really takes a dump on the game.

depends on context. you're probably referring to "CAP" tables, which are tables on which the deepest stack you can start a hand with is 20bb. perpetually short-stacked tables.

From what I hear, unless you're playing 100nl+ CAP is pointless. Not sure why, maybe due to rake.
