A poker hand is a conversation between the players. The words are the bets/checks and folds (taking into account body language/ thinking time etc. which doesnt apply to online poker). This is the crucial point in the hand. Your opponent has been telling you all the way that they need help. The turn card comes and completes 2 possible draws (straight and flush) and you need to decide if it is the help they have been waiting for. So far everything their bets have said has been consistent. There is no evidence that they have changed their mind or lied so believe them. The other point is that you have shown strength the whole way representing at least top pair. I cant remember who said it but "If you have the obvious hand then believe the raiser". I think you have to fold this without a read.

I guess just practise. If you train yourself to wait for 10 seconds before making any decision (including foldoing 72o preflop/ raising AA preflop etc.) then that might do the job. It will annoy the hell out of other players though.
As for plugging leaks in general take a look at this thread and read everything you find by aokrongly.
