That trick is normally done with a fake thumb that slips over your real thumb with the silk hankerchief inside. So...does she stick the fake thumb up her you-know-what or does she have a hankie up in there all the time?

She was playing to the audience and not to the camera. Rather well I might add.
Stage acting and make-up tends to be exagerated and not look as good close up.
