That's a bad one all right. I was dealing a NL game at the casino and end up with a heads up situation with a flop of:
Joe has $1000
Wilma has $1500
  
Joe bets out 100, Wilma smooth calls.
Turn
Joe bets out 100, Wilma smooth calls again.
River
Joe bets out 200, Wilma puts him all in.
Joe stares at her and asks "What do you have? Pocket 7's?"
He calls.
Joe shows:
 
Wilma shows:
 
She had to have exactly those two cards to beat him, and that's exactly what she had. He figured it was a split pot at worst and had to call.
It's not just online, these things happen.

I don't think the casino deal was a bad beat, though. She had him the whole time.
